This forms tells us who in your Club/Unit will receive membership information. This includes new member information, renewal information, dues report, and audit list. Multiple members from a unit can be on your communication list providing it is officer approved.

If we do not have a Communication form on file, membership information will default to the Club/Unit President, Treasurer and Membership Chair.
