This forms tells HQ who in your Club will receive membership information. This includes new member information, renewal information, dues report, and audit list. Multiple members from a Club can be on your communication list providing it is officer approved.

If HQ does not have a Communication form on file, membership information will default to the Club President, Treasurer and Membership Chair.
